Parents, Caregivers & Youth

Find an EPIC-IC Medical Home practice for your child!

  • A family-centered medical home is not a building, house, hospital, or home healthcare service. It is an approach to providing comprehensive primary care.
  • In a family-centered medical home the pediatric care team works in partnership with your child and your family to assure that all of the medical and non-medical needs of your child are met.
  • Through this partnership the pediatric care team can help your family access, coordinate, and understand specialty care, educational services, out-of-home care, family support, and other public and private community services that are important for the overall health of your child and family.
  • The American Academy of Pediatrics (AAP) developed the medical home model for delivering primary care that is accessible, continuous, comprehensive, family-centered, coordinated, compassionate, and culturally effective to all children and youth, including those with special health care needs.

Hi Lori, It's only be over a year and I never replied back to you. Better late than never. Yes, my boys, ages 9 and 7. 9 y.o. has Asperger's and is very defiant (got suspended from school 4 times this year) and the 7 y.o. is NT and is a very good brother and son. Both are good kids but we definately have our moments with the 9 y.o. Jen

  • Don't mind your asking. My oldest has Aspergers, ADHD, Anxiety Disorder, Oppositional Defiance Disorder, GERD, Hiatal Hernia, Erosive Esophagitis. My middle son has Autism, Mild MR, Tourette's, Hiatal Hernia and GERD as well. My youngest has a dairy allergy, but so far aside from some minor sensory and oral motor issues, he isn't showing signs of being on the spectrum and didn't qualify for services since turning 3. I have inoperable cavernous angiomas in the brain and spinal cord which make all this even more difficult to deal with. Thank God for my healthy husband.

  • That is me, my husband and our 3 sons. The one on the far left is 7. He has severe apraxia of speech with a phonological disorder and ADHD. In the middle is our 9 year old who has Asperger's and something that seems like IED. It sounds like your 15 year old has similar issues. Our 6 year old on the far right is a typical Kindergartener. :)

  • Hi momof5!! My 8 yr old has a cleft palate that has a layer of skin over it so it wasn't diagnosed until he was 2 and had to get his tonsils and part of his adenoids out that we even knew he had a cleft palate. They couldn't take all of his adenoids out due to the cleft palate. They said if they removed all of the adenoids then everytime he ate or drank it would come out of his nose. I was shocked to find out he even had a cleft palate, but it did explain a lot about why I couldn't nurse him and all of his ear infections!! He still has tubes in both of his ears due to ear infections!! The Klippel-Feil syndrome is where the bones in his neck and shoulder are fused together so his one shoulder is always raised. We just have to watch for curvature of the spine when he is older due to the syndrome!! Did your son lose his hearing when he was older or was he born with hearing loss? I think my son Seth has a mild hearing loss due to all of his ear infections. It sounds like you really have your hands full with your children! Maybe we can keep in touch !

  • Thank you for the welcome! The boy in the picture is my son; he is 11 in that photo, but looks pretty much the same now, at nearly 14, as he did back then. He was all smiles & very excited that day b/c it was the day we brought home his new wheelchair, which you can see in the photo.
