  • A family-centered medical home is not a building, house, hospital, or home healthcare service. It is an approach to providing comprehensive primary care.
  • In a family-centered medical home the pediatric care team works in partnership with your child and your family to assure that all of the medical and non-medical needs of your child are met.
  • Through this partnership the pediatric care team can help your family access, coordinate, and understand specialty care, educational services, out-of-home care, family support, and other public and private community services that are important for the overall health of your child and family.
  • The American Academy of Pediatrics (AAP) developed the medical home model for delivering primary care that is accessible, continuous, comprehensive, family-centered, coordinated, compassionate, and culturally effective to all children and youth, including those with special health care needs.

  • Welcome, I have a son with RAD (reactive attachment disorder) went though many trauma at a young age. I adopted him and have had an on going battle of wills. His out pt therapist took a new job and we are now going to meet a new therapist.He doesn't do well with change. He acts out instead of verablizing his problems. This causes many problems in our home and with neighbors,friends.I ask myself everyday what can I do differently,or am I doing more harm then good on days that he is at his worst.No one is there to tell or offer any helpSo I take one day at a time and enjoy the good days and cry alot on the bad days.
