  • A family-centered medical home is not a building, house, hospital, or home healthcare service. It is an approach to providing comprehensive primary care.
  • In a family-centered medical home the pediatric care team works in partnership with your child and your family to assure that all of the medical and non-medical needs of your child are met.
  • Through this partnership the pediatric care team can help your family access, coordinate, and understand specialty care, educational services, out-of-home care, family support, and other public and private community services that are important for the overall health of your child and family.
  • The American Academy of Pediatrics (AAP) developed the medical home model for delivering primary care that is accessible, continuous, comprehensive, family-centered, coordinated, compassionate, and culturally effective to all children and youth, including those with special health care needs.

  • hi thankyou.....it's been a while since i have been on a site like this one.....i was without a computer for quite sometime....you really don't know how much you will miss something til it's gone. it's nice to connect with other people who know what it's like to have children with special needs and although we all have different challenges with our children there is an understanding that wecan have for each otherand we no longer feel quite as alone. take care jennifer
