The National Center for Medical Home Implementation (NCMHI) hosted the first of a three-part webinar series focusing on patient- and family- centered care coordination and its benefits for children, families, and clinicians.  The webinar will expand on information, tools, and resources presented in the AAP Policy statement, "Patient- and Family- Centered Care Coordination: A Framework for Integrating Care for Children and Youth Across Multiple Systems."

The National Center for Medical Home Implementation is hosting an information webinar on Fetal Alcohol Spectrum Disorders (FASDs).  This webinar will include an overview of an algorithm for evaluation of FASDs that can be used as a tool for diagnosis and referral in pediatric primary care medical home practices.
